Just to clarify, basically the only people currently eligible for Medicaid in North Carolina are pregnant women, families with minor children, and individuals who are 65 and over, blind or found (by some very stringent criteria) to be disabled. In addition, the applicant must meet the income guidelines you refer to.
So if you're a person under 65 who is not disabled and doesn't have children, pretty much the only Medicaid coverage you can get is for birth control, even if you meet the income guidelines.
It appears from some of the information that's being unearthed that the shooter was closeted gay himself. This seems to be the pattern for the most virulent homophobics. Perhaps this is the case for Rexford Alexander as well.
